Whether you’re just starting dialysis, preparing for transplant, or trying to navigate the healthcare system, this resource can help you feel more prepared and less alone.
The packet can be downloaded at no cost, and we encourage you to share it with friends, family, or caregivers who want to understand what you’re going through.
Doctor Directory
Finding the right care team can make all the difference when you’re living with kidney disease. Our Doctor Directory is a growing list of nephrologists, transplant providers, and other kidney-friendly professionals who understand the unique needs of young adults and their spouses.
What sets this directory apart is that it’s built on lived experience. Each provider listed has been recommended by someone who’s walked this path themselves a patient or spouse who knows firsthand what it’s like to navigate dialysis, transplant, and the challenges of long-term care.
We’re continually expanding the directory to include specialists, clinics, and supportive providers across the region. By sharing these recommendations, we hope to make it easier for you to find care that is compassionate, knowledgeable, and respectful of your journey.
You can browse the directory to explore providers others have found helpful and you can also contribute by submitting your own recommendation. Every new name helps another young adult or spouse feel less alone and more supported in choosing their care.
